In September, 2015 Horizon Health Network became aware of a pamphlet prepared and distributed by Donna Collins of Crystal Clear Transitional Care. The pamphlet design could suggest it was prepared by or for Horizon Health Network.

Horizon Health Network wishes to advise that neither Ms. Collins nor Crystal Clear Transitional Care is authorized to prepare or distribute any material in any format on behalf of Horizon Health Network. Ms. Collins is not an employee, agent or contractor with Horizon Health Network.  Horizon Health Network has no association or business dealings with Ms. Collins or Crystal Clear Transitional Care, nor does it accept or endorse any material prepared or distributed by Ms. Collins. 

Ms. Collins has been requested to cease publication and distribution of any material which would suggest to the public that it is material produced or endorsed by Horizon Health Network.
