COVID-19 Assessment Centres by appointment
If you are unable to attend your COVID-19 test at one of Horizon's COVID-19 Assessment Centres due to weather, please resubmit your request by visiting www.gnb.ca/coronavirus or call Tele-Care 811. Once you resubmit, you will be contacted with a new appointment time.
COVID-19 Assessment Centres across Horizon offer appointments for patients exhibiting mild to moderate symptoms of Coronavirus (COVID-19).
Appointments are provided following a triage completed by Tele-Care 811.
Assessment centres will help to reduce visits to emergency departments and minimize the risk of transmission.
These are not walk-in clinics. They will complete COVID-19 screening and testing by appointment only.
Assessment centres are set up in Moncton, Saint John, Miramichi, Fredericton and Upper River Valley.
If you are displaying mild or moderate symptoms of COVID-19, complete the self-assessment by visiting www.gnb.ca/coronavirus or call Tele-Care 811.
In an emergency, or if you are experiencing severe symptoms, please call 911 or call ahead to your local Emergency Department so they can prepare for your arrival.

How do I access the assessment centre?
- Individuals can access the assessment centre by contacting Tele-Care 811.
- Following an assessment by Tele-Care, if/when the client requires an in-person assessment a referral will be sent to a COVID-19 Assessment Centre; the staff at the assessment centre will contact the individual to schedule an appointment, this should be completed within 24 hours, but it will depend on the number of referrals.
What are the hours of operation?
- Every effort will be made to screen as many individuals as possible each day. The hours may vary as it will be dependent on the volume of referrals received. These assessment centres are not walk-in clinics, they are accessed by appointment through 811 only.
- Please check in regularly for updates on our website and social media channels.
What health care services are provided?
- Individuals will receive access to screening, be directed to further care if/when needed as well as provided with evidence-based resources and information about COVID-19.
Are there drive-throughs?
- Drive throughs have been set up in Moncton, Fredericton and Miramichi for initial screening. If/when a client requires more intensive screening they will be asked to enter the assessment centre. We are looking at this option for the other assessment sites.
How is it staffed?
- There will be a health care provider that is able to do an assessment, provide medical advice and guidance, and complete the screening test. The health care team may include a physician, nurse practitioner, Registered Nurse or a Licensed Practical Nurse.
What's the difference between self-isolation and self-monitoring related to Coronavirus (COVID-19)?
You self-monitor when you:
- Have no symptoms
- Have had a possible exposure to COVID-19 in the last 14 days

How do you self-monitor?
Monitor yourself for 14 days for one or more symptoms of COVID-19:
Go about your day but remember to follow provincial guidelines related to the State of Emergency.
If you need to go in public, maintain a two metre (six foot) distance between yourself and others.
If you develop symptoms, isolate yourself from others (including your household) immediately.
If you are displaying mild or moderate symptoms of COVID-19, complete the self-assessment by visiting www.gnb.ca/coronavirus or call Tele-Care 811. They may refer you to one of the Community Assessment Centres for testing.
How do you self-isolate?
Stay home and monitor your symptoms, even if mild, for 14 days.
Avoid contact with others (including your household) to prevent the spread of disease in your home.
Here are some helpful items to have on hand during self-isolation:
- One-time use masks
- Disposable gloves (do not re-use)
- Paper towel and tissues
- Waste basket with disposable, plastic liner
- Thermometer
- Medication to reduce fever (ibuprofen or acetaminophen)
- Alcohol-based sanitizer
- Arrange to have groceries delivered
Keep other members of your household safe and healthy by cleaning and disinfecting surfaces that you touch often, such as toilets, bedside tables, doorknobs, phones and television remotes.
Why is Perth-Andover the only location accepting clients without an appointment?
Drop-in service at the Perth-Andover location is a temporary measure to encourage testing at for residents in the area at the new, temporary COVID-19 Assessment Centre.
What do I do when I arrive for a drop-in appointment at the Perth-Andover Assessment Centre?
First, you arrive at the Perth-Andover Baptist Church at 194 Barrett St. with your New Brunswick Medicare card. Once you arrive, drive towards the Horizon signage and staff will direct you where to go. You'll remain in your vehicle for the entire process.
